Housing Supports
Services are designed for Medicaid-eligible adults (18+) in New Jersey who are currently homeless or at risk of losing their housing, enrolled in one of our approved MCO plans.
No. All housing support services are provided at no cost to eligible Medicaid members. Services are funded through NJ FamilyCare via your MCO.
Pre-Tenancy support helps individuals who do not currently have stable housing find and move into a home. Tenancy Sustaining support helps those already housed to remain stably housed long-term.

Support Brokerage empowers individuals to self-direct their own care. A Support Broker helps plan, budget, and manage your personal care team according to your preferences and goals.
